Continuum of Care
A Continuum of Care in the healthcare field is described as a range of healthcare services that are aimed at providing differing levels of services from “start to finish” in a seamless manner.
For example, at WestBridge, this could mean that an individual may be introduced to WestBridge
services by utilizing our Nationwide Consultation/Evaluation. If an individual is a good fit for the program and he or she is interested in pursuing services, they then have the option to begin their recovery at our male residential facility, The Commons, move into the community and continue their recovery with Care Management services for a short or long term period. Utilizing WestBridge services enables the individual to begin their recovery with us at any level of care; it is not necessary for one to receive a clinical level of care as a prerequisite of another.
We subscribe to the well known belief that relationships are the foundation to a healthy life. This full Continuum of Care that WestBridge provides is meant to foster the relationships we build with our participants and families. WestBridge is meant to be a setting that our families and participants feel safe and that they are building relationships and skills that are designed to meet life goals and maintain good health for the long term.
